Después de las palabras de la Virgen Santísima, cuenta la hermana Sor Catalina Labouré, que alrededor de la cabeza de la Virgen se formó un círculo o una aureola con estas palabras:
"Oh María sin pecado concebida, ruega por nosotros que recurrimos a Ti". Y una voz dijo a Catalina:
"Hay que hacer una medalla semejante a esto que estás viendo. Todas las personas que la lleven, sentirán la protección de la Virgen", y apareció una M, sobre la M una cruz, y debajo los corazones de Jesús y María.
Esa imagen es la que hoy está en la Medalla Milagrosa.
