La Acantosis Nigricans (acanto proveniente del griego espina y nigricans proveniente del latín negro) es una manifestación cutánea fácilmente reconocible que se caracteriza por la presencia de placas hiperpigmentadas (oscurecidas) aterciopeladas en los pliegues corporales, generalmente simétricas y frecuentes en la parte posterior y lateral del cuello, axilas, región inguinal, pliegues sub-mamarios y periné.
🟡 ¿Qué causa la Acantosis nigricans?
Puede estar causada por diversos trastornos sistémicos. Lo más habitual es que ocurra como consecuencia de la resistencia a la insulina que acompaña muchas veces a entidades como Diabetes mellitus tipo 2, síndrome de ovarios poliquísticos y la obesidad.
Además, aunque es muy inusual, estudios científicos han asociado la Acantosis nigricans a la presencia de neoplasias malignas, ubicadas en el aparato digestivo con mayor frecuencia.
🟡 ¿Qué hacer si tiene Acantosis nigricans?
Acudir al médico. Puede consultar con un médico general o médico internista, que posterior a su evaluación y seguimiento pueda determinar el origen, dar inicio al tratamiento o referirle a la especialidad más adecuada.
🟡 ¿Tiene tratamiento esta manifestación?
El tratamiento está dirigido a la causa subyacente, su facultativo le realizará las indicaciones pertinentes y, en ocasiones, podría complementarse con el tratamiento tópico de esta dermatosis.
Acanthosis Nigricans (acanthosis from Greek spina and nigricans from Latin black) is an easily recognizable cutaneous manifestation characterized by the presence of hyperpigmented (darkened) velvety plaques in body folds, generally symmetrical and frequent on the back and sides of the neck, armpits, inguinal region, sub-mammary folds and perineum.
🟡 What causes Acanthosis nigricans?
It can be caused by various systemic disorders. Most commonly it occurs as a consequence of insulin resistance that often accompanies conditions such as type 2 diabetes mellitus, polycystic ovarian syndrome and obesity.
In addition, although it is very unusual, scientific studies have associated Acanthosis nigricans to the presence of malignant neoplasms, most often located in the digestive tract.

See your doctor. You can consult a general practitioner or internist, who after evaluation and follow-up can determine the origin, start treatment or refer you to the most appropriate specialty.
🟡 Is there any treatment for this manifestation?
The treatment is directed to the underlying cause, your physician will give you the pertinent indications and, sometimes, it could be complemented with the topical treatment of this dermatosis.
Hace unos meses estaba trabajando en un colegio realizando los exámenes médicos prevacacionales al personal de la institución educativa, haciendo una anamnesis y el examen físico completo a cada uno de los docentes, obreros y coordinadores, complementándolo con los exámenes de laboratorio que les incluía el servicio de salud y seguridad laboral.
Fue así como me topé con una señora de 51 años, con Obesidad clase I (según su índice de masa corporal y la clasificación de la Organización Mundial de la Salud), que no refería alguna sintomatología durante el interrogatorio, pero fue durante el examen físico que se reanudaron nuevas interrogantes de mi parte.
A few months ago I was working in a school performing pre-holiday medical examinations to the staff of the educational institution, making an anamnesis and complete physical examination to each of the teachers, workers and coordinators, complementing it with laboratory tests that included the occupational health and safety service.
This is how I came across a 51 year old woman, with class I obesity (according to her body mass index and the World Health Organization classification), who did not refer any symptomatology during the interrogation, but it was during the physical examination that new questions were raised by me.
Se observa claramente la marcada hiperpigmentación en región de los pliegues submamarios, abdominales, genitales e inguinales.
The marked hyperpigmentation in the region of the submammary, abdominal, genital and inguinal folds is clearly observed.
Resulta que la paciente tenía muchos años sin acudir a un médico, no precisó cuántos, pero eran los suficientes para obviar el deterioro de su salud en general, puesto que efectivamente antes no tenía esas “manchas”. También, ella refería que aumentó de peso en los últimos años, al igual que su hijo adolescente que también estaba iniciando a presentar “esas manchas” y no pensó que fuese alguna alteración de la cual preocuparse. Negaba antecedentes personales de Hipertensión, Diabetes o alguna otra morbilidad. Curiosamente, los exámenes de laboratorio (hemograma, úrea, creatinina, glicemia en ayunas y perfil lipídico) resultaron estar dentro del rango normal.
Lo cierto es que le explique detalladamente a la paciente lo que ese hallazgo incidental podía significar, le recomendé cambios en su estilo de vida y que lo más pronto posible asistiera a un médico internista para hacer seguimiento de su caso, pues aunado a esos signos, estaba comenzando a presentar cambios en la piel de sus tobillos, siendo la fase previa al desarrollo de unas úlceras varicosas.
It turns out that the patient had not seen a doctor for many years, she did not specify how many, but they were enough to obviate the deterioration of her general health, since she did not have these "spots" before. She also mentioned that she had gained weight in the last few years, as had her adolescent son, who was also beginning to show "those spots" and did not think that it was an alteration to worry about. She denied any personal history of hypertension, diabetes or any other morbidity. Curiously, the laboratory tests (hemogram, urea, creatinine, fasting glycemia and lipid profile) were within the normal range.
The truth is that I explained in detail to the patient what this incidental finding could mean, I recommended changes in her lifestyle and that she should see an internist as soon as possible to follow up on her case, because in addition to these signs, she was beginning to present changes in the skin of her ankles, being the previous phase to the development of varicose ulcers.
