El día de hoy quisiera compartir con ustedes una de mis más desagradables y dolorosa aflicción de salud, esta experiencia ha Sido quizás la más dolorosa y llena de incertidumbre que he padecido, pues me he visto incurso en diagnósticos erróneos los cuales han hecho que la enfermedad se haya mezclado con la angustia y estrés.

Today I would like to share with you one of my most unpleasant and painful health afflictions, this experience has been perhaps the most painful and full of uncertainty that I have suffered, because I have been involved in misdiagnoses which have made the disease has been mixed with anguish and stress.

Desde el día 30 de diciembre del pasado año 2022. Había estado padeciendo de molestia en la zona anal, molestias que se podrían describir como pinchazos y picor. Sin embargo, estas molestias se las estuve atribuyendo a las hemorroides dado que en otras oportunidades he sufrido de hemorroides. Cómo paciente proctológico ya conocía alguno que otro método para calmar esa desagradable sensación, baños de asiento con agua tibia y aplicación de alguna pomada antihemorroidal, tal como bargonil la cual contiene un poco de mentol, Lidocaína e hidrocortisona los cuales son muy eficientes al momento de presentar dolor o molestia.

Since December 30 of last year 2022. I had been suffering from discomfort in the anal area, discomfort that could be described as prickling and itching. However, I had been attributing these discomforts to hemorrhoids since I had suffered from hemorrhoids on other occasions. As a proctological patient I already knew some other method to calm that unpleasant sensation, sitz baths with warm water and application of some anti-hemorrhoidal ointment, such as bargonil which contains a little menthol, Lidocaine and hydrocortisone which are very efficient at the moment of presenting pain or discomfort.

Cómo no tenía los medicamentos a la mano decidí contactar con una red de farmacias de mi ciudad, la cual brinda asistencia telefónica vía WhatsApp y también cuentan con un estupendo servicio de entregas a domicilio totalmente gratuito (Delivery). Fue así como contacte con el farmaceuta y solicite asistencia. Él me recomendó una pomada llamada Bargonil la cual efectivamente ya había usado anteriormente, notolac para el dolor y unos supositorios vía anal llámanos Anomex los cuales también contenían analgésicos.

As I did not have the medicines at hand, I decided to contact a network of pharmacies in my city, which provides telephone assistance via WhatsApp and also has a great home delivery service totally free of charge (Delivery). I contacted the pharmacist and asked for assistance. He recommended me an ointment called Bargonil which I had used before, notolac for pain and some anal suppositories called Anomex which also contained analgesics.

Luego de escuchar todas las indicaciones del farmaceuta. Realice la orden, pague vía transferencia y a los 15 minutos me trajeron todos los medicamentos necesarios. En ese mismo momento Comencé mi tratamiento.

After listening to all the pharmacist's instructions. I placed the order, paid via wire transfer and within 15 minutes they brought me all the necessary medications. At that very moment I started my treatment.

Ya para el día 3 de enero me sentía un poco mejor la recuperación, aunque no era significativa si era un poco notoria una leve mejoría, así que decidí ir al día siguiente al médico. Al llegar a la clínica supe que el gastroenterólogo que conoce mi historia médica se encontraba aún de vacaciones, así que tuve que examinarme con otro gastroenterólogo, en la consulta le conté todo lo que me dolía y todo lo que había realizado para calmar el dolor.

By January 3rd I felt a little better recovery, although it was not significant if it was a little noticeable a slight improvement, so I decided to go the next day to the doctor. When I arrived at the clinic I found out that the gastroenterologist who knows my medical history was still on vacation, so I had to be examined by another gastroenterologist, in the consultation I told him everything that hurt me and everything I had done to relieve the pain.

En la revisión médica se me realizó un examen de tacto para ver qué tan grave eran las hemorroides y admito que al realizarme ese procedimiento sentí el dolor más terrible y traumático de mi vida, fue literalmente una sensación como si me hubiesen apuñalado con un punzón el recto. Era insoportable el dolor y el médico concluyó que se trataba de una fisura anal con presencia de hemorroides. El protocolo médico fue recetarme:

• Pomada de Diltiazen al 2%, 3 veces al día.

• Diosmina en tabletas de 500mg para ayudar a la circulación sanguínea.

• Lidocaína en gel. (Antes y después de cada defecación).

At the medical examination I was given a touch test to see how serious the hemorrhoids were and I admit that when I underwent this procedure I felt the most terrible and traumatic pain of my life, it was literally a sensation as if I had been stabbed with an awl in my rectum. The pain was unbearable and the doctor concluded that it was an anal fissure with the presence of hemorrhoids. The medical protocol was to prescribe:

• Diltiazen 2% ointment, 3 times a day.

• Diosmin 500mg tablets to help blood circulation.

• Lidocaine gel (before and after each bowel movement).

Antes de ir a casa pasé por algunas farmacias para comparar precios y buscar un poco de economía y así comprar todos los medicamentos, admito que ese trayecto fue muy doloroso, pues el dolor había vuelto y con él todos los síntomas anteriormente descritos. Al llegar a casa realice un baño de asiento para poder relajar un poco la zona y comenzar el nuevo tratamiento.

Before going home I went to some pharmacies to compare prices and look for a little economy and thus buy all the medicines, I admit that this journey was very painful, because the pain had returned and with it all the symptoms described above. When I got home I took a sitz bath to relax the area a little and start the new treatment.

Los días pasaron y no note ninguna mejoría, fue así como el día 7 de enero tuve una crisis de dolor acompañada con espasmos en la zona anal, lo cual literalmente me estaba volviendo loco. Como pude salí a la sala de emergencias del hospital a volverme a chequear. La médico de guardia que me atendió me coloco una ampolla de diclofenac intramuscular para calmar aquel infernal dolor, posteriormente ratifica el mismo tratamiento del gastroenterólogo que me había atendido días atrás. Sin embargo, algo dentro de mí me decía que ese tratamiento no me estaba haciendo ningún efecto, sin embargo, quise tener fe en que si mejoraría.

The days passed and I did not notice any improvement, so on January 7th I had a pain crisis accompanied by spasms in the anal area, which was literally driving me crazy. As I could, I went to the emergency room of the hospital to be checked again. The doctor on duty who attended me gave me an ampoule of intramuscular diclofenac to calm that infernal pain, later ratifying the same treatment of the gastroenterologist who had attended me days before. However, something inside me told me that this treatment was not having any effect, but I wanted to have faith that I would get better.

El día de ayer, mientras estaba colocándome la pomada, pude notar que en la zona anal tenía un bulto muy grande, la temperatura era algo cálida y se percibía al tacto mucha inflamación. Esto hizo que el estrés nuevamente se apoderara de mí así que, con mucha pena, decidí tomarme una fotografía, para enviársela a una amiga que es médico proctólogo en Cuba. (Fotografía que por obvias razones no puedo colocar acá).

Yesterday, while I was applying the ointment, I noticed that I had a very large lump in the anal area, the temperature was somewhat warm and there was a lot of inflammation to the touch. This made me feel stress again, so I decided to take a picture to send it to a friend of mine who is a proctologist in Cuba (picture that for obvious reasons I cannot post here).

Luego de ver detenidamente esta fotografía pude observar como en uno de los costados del bulto se observaba algo blanco con presencia de puntos rojos, algo muy parecido a un absceso a punto de reventar.

After looking closely at this photograph I could see how on one of the sides of the lump there was something white with red dots, something very similar to an abscess about to burst.

Ella al ver esta fotografía y al oír todo lo anteriormente narrado me dice "Oye, esto no es hemorroides. Eso es un absceso perianal." Cuando ella me dice eso dije dentro de mí este si es el verdadero diagnóstico. Anoche mismo me recetó cefadroxilo de 500 mg, baños de asiento con agua tibia e ibuprofeno + tiocolchicosido para el dolor. Increíblemente, luego de hablar con ella, el absceso se comenzó a drenar por si solo y poco a poco el dolor iba desapareciendo.

She, upon seeing this photograph and hearing all of the above, says to me, "Hey, this is not hemorrhoids. That is a perianal abscess." When she tells me that I said inside me this if the true diagnosis. Just last night she prescribed me cefadroxil 500 mg, warm water sitz baths and ibuprofen + tiocolchicoside for the pain. Incredibly, after talking to her, the abscess began to drain on its own and little by little the pain was disappearing.

Ese momento fue algo gratificante, pues había dado con la patología correcta, ya sabía cuál era el tratamiento ideal, pero sobre todo el dolor estaba desapareciendo poco a poco. Así que fui a lavarme un poco con jabón neutro y a colocarme un poco de yodo para esterilizar la herida. Esta mañana, apenas amaneció, mandé a mi esposa a qué me buscará en la farmacia los antibióticos y demás medicamentos para comenzar el nuevo tratamiento.

That moment was somewhat gratifying, because I had found the right pathology, I knew what the ideal treatment was, but above all the pain was gradually disappearing. So I went to wash myself a little with neutral soap and put some iodine to sterilize the wound. This morning, as soon as it was light, I sent my wife to the pharmacy to get me the antibiotics and other medicines to start the new treatment.

De todo esté trauma, me ha quedado de experiencia mejorar mis hábitos de alimentación, tener un consumo alto de agua, incorporar fibras a mi dieta para evitar el estreñimiento. Pero también a buscar siempre segundas opiniones médicas, ya que las primeras no siempre son las acertadas.

From all this trauma, I have learned to improve my eating habits, to have a high consumption of water, to incorporate fiber to my diet to avoid constipation. But also to always seek second medical opinions, since the first ones are not always the right ones.

Nunca antes yo había padecido dolores similares a este, de verdad es algo que no se lo deseo a nadie, pero también es necesario conocer un poco más de este tipo de aflicciones y que hábitos pueden desencadenar una situación como está. El dolor de hemorroides es algo insoportable, pero un absceso anal es algo mucho más doloroso y tormentoso, esta es una enfermedad que puede darle a cualquier persona y en el momento menos esperado, así que la mejor arma que podemos tener en contra de ella es cuidar bien de nuestra alimentación.

I had never suffered pain similar to this before, it is really something that I do not wish to anyone, but it is also necessary to know a little more about this type of affliction and what habits can trigger a situation like this. Hemorrhoid pain is something unbearable, but an anal abscess is something much more painful and tormenting, this is a disease that can hit anyone and at the least expected time, so the best weapon we can have against it is to take good care of our diet.

Así que continuaré mi tratamiento al pie de la letra y manteniendo un reposo ideal, ya que la recuperación por este tipo de patologías es un poco lenta y en algunos casos puede requerir intervención quirúrgica cuando no se atiende a tiempo.
